Training for the Boston Marathon? Mass Bay Spine & Sport PT Shares Tips for Avoiding Injury

With the Boston Marathon around the corner, Brendan Carman, MPT, ATC, Director of Rehabilitation at Mass Bay Spine & Sport Physical Therapy (MBSSPT), shares his insight on the causes of running injuries and how to avoid them.

Carman, who oversees the state-of-the-art orthopedic outpatient physical therapy practice in Marshfield, has treated countless injuries in runners who are training for marathons and half-marathons over the years.

Below, Carman outlines the causes of certain injuries when training:

Over-training. Achilles tendonitis (painful inflamation of the achilles tendon), Plantar Fasciitis (which exhibits as heel pain) and shin splints (front of leg pain) are all serious injuries resulting from over-training. When training for the Boston Marathon, runners often focus more on endurance and stamina than on overall strengthening of the entire lower body. The first indicator of over-training is aches and pains.

“When a patient presents with aches and pains, its best to deal with it sooner than later. If caught early, one to two days of rest is usually all that is needed. But, aches and pains that are ignored can become more significant. The tissues break down and can become an injury if not addressed promply. Then, it can take weeks to resolve the issue,” explained Carman.

Achilles tendonitis in particular can get progressively worse and result in degeneration of the tendon if left untreated.

Abrupt changes in regimen. Adding five miles to that daily run or altering the pace are actually abrupt changes in regimen that can spark an injury. It will become worse if symptoms start and a runner tries to work through it. Achilles tendonitis is most often the result.

Poor footwear. Old, worn, broken down sneakers offer no support. Wearing the improper size sneaker also creates problems. If the size is too large, then there is an increase in the lever arm so force through the calf muscle is increased and the tendons break down. Improper footwear can also cause Achilles tendonitis. It is generally recommended that running shoes be changed for every 350 to 500 miles trained, depending on body type and training style.

Muscle imbalance. The fundamental problem most of the time is muscle imbalance or weakness that doesn’t get addressed. Training goes beyond just running. Training should include a strengthening and stretching program for the whole lower half of the body, including the muscles of the core.

According to Carman, “At Mass Bay Spine & Sport, we focus on a lot of functional movement type activities with empahsis on balance, that engage the muscles all the way through the hip. This is a significant component. Weak hips will cause problems with knees and ankles because they are not stabilized. Hip weakness is often a key contributing factor to the onset of shin splints and Plantar Fasciitis.”

Mass Bay Spine & Sport Physical Therapy Staff Laces Up for St. Patrick’s Day 5K Road Race

The staff of Mass Bay Spine & Sport Physical Therapy (MBSSPT) will kick up their heels, literally, on St. Patrick’s Day as they participate in second annual St. Patrick’s Day 5K Road Race in Marshfield on Saturday, March 17, 2012.

MBSSPT, a state-of-the-art orthopedic outpatient physical therapy practice based in Marshfield, has formed a team to run in the 5K Road Race which benefits the Marshfield Education Foundation.

The race begins at 10 a.m. at the Venus II restaurant located at 277 Ocean Street.

“We have quite a few runners on staff. The weather has been mild so it has made training for the race a little easier than it normally would be. We’re all excited to put our best foot forward for this very worthy cause,”noted Brendan Carman, MPT, ATC, Director of Rehabilitation, who is the team’s captain.

The staff also ran as a team last November in the Kingston Turkey Trot, which raised funds to promote the awareness of ovarian cancer.

Mass Bay Spine & Sport Physical Therapist Offers 5 Tips for Avoiding Winter Injuries

The poet Robert Frost said, “You can’t get too much winter in the winter”. According to Brendan Carman, MPT, ATC, Director of Rehabilitation at Mass Bay Spine & Sport Physical Therapy (MBSSPT), if you suffer a strain or injury brought on by too much shoveling, winter activities or from slipping on an icy patch in your driveway – then it is too much winter.

MBSSPT is a state-of-the-art orthopedic outpatient physical therapy practice based in Marshfield.

Carman offers five tips for avoiding winter injuries:

Wear the proper gear. Snow, slush and ice are slip hazards that should be taken seriously. MBSSPT has worked with a number of patients who have injured their wrists, ankles, shoulders, knees and backs from falls in bad weather. One of the best ways to prevent falls is by wearing the proper winter gear, such as sturdy boots with rubber soles. If you participate in winter sports, be sure to wear the appropriate attire and protective gear, especially if you have suffered an injury previously.

Learn the correct technique first. If you’re a “newbie” at any winter sport, from skiing and snowboarding to ice skating, or if you’re switching from one sport to another, understand that posture, balance and weight distribution are different for each sport. Proper technique is imperative to avoid strain or injury that could set you back for weeks or months at a time. Learn before you leap into a new sport. Take lessons through a certified instructor to help ensure you avoid injury.

Choose the right tools. Snow is inevitable in New England and so is shoveling. In a perfect world, we would all be able to employ a snow removal service for even the smallest amount of snow. Selecting the right “tool” for shoveling is key. Choose a shovel that is sturdy and lightweight. Or, opt for a “pusher” shovel with a blade that looks like a snow plow. This type of shovel is very easy on the back because you literally push the snow to the side, as opposed to picking it up. Got ice? Ice melt is one of the best “tools” around for eliminating hazardous ice. Not a fan of ice melt? Sand or kitty litter can provide traction on slippery areas.

Keep your balance. As with winter sports, balance is key when shoveling. To avoid injury while shoveling, bend from the hips and knees to shovel. Tightening your abdominal muscles while you lift the shovel will help support your back. Take care not to twist your back to throw the snow.

Don’t overdo it. Fatigue is the enemy. If you’ve been playing sports or shoveling for an hour or more without a break, you risk overexertion, which leads to pulled muscles. Working tired muscles can lead to serious strain or injury. Don’t skip breaks. If you’re shoveling, you should take a break at least every 15-20 minutes.

According to Carman, “Low back pain, sciatica, herniated discs, muscle strains and shoulder pain are the most common winter related conditions we treat. Following our five key tips will help you avoid strain and injury.”
